What Makes Men Sexy Today? Inside the Rise of Male Aesthetics
As men embrace non-surgical enhancements like jawline fillers and penile injectables, Lift Aesthetic Clinic leads the way in shaping confidence and masculinity in the age of subtle aesthetics
Breaking the taboo
Men have traditionally approached aesthetic clinics with caution. Curious, sometimes insecure, but hesitant. Enhancement was often framed as indulgent or unnecessary, a domain dominated by women and celebrity culture. For many men, even asking about a procedure felt like crossing an invisible line.
That line is fading.
At Lift Aesthetic Clinic in the Philippines, male patients now make up about 55 to 60 percent of the client base. The clinic is co-owned by aesthetic physician Dr. Cyril Mitchel Agan and creative director Jan Raymond Conadera.

Dr. Cyril specializes in non-surgical injectable treatments, including fillers, toxins, and threads, while Jan, an IT specialist by training, oversees operations and marketing. Together, they have positioned Lift as a space where male enhancement is treated as structured and medically guided self-improvement.
“Globally, about 90 percent of the aesthetics market is female-driven,” Dr. Cyril says. “Before, aesthetics was really seen as a woman’s world. In recent years, more and more men feel it is acceptable to undergo procedures. But of course, the goal is still to look natural. That’s the main change.”
Jan recalls how different the climate was a decade ago. “Ten years ago, everything from non-surgical to surgical was quite taboo for men,” he says. “They wanted to keep it private.” When conceptualizing Lift, he asked a practical question: where could men go for high-quality procedures and still feel comfortable?
“I wanted a place where men would feel comfortable doing these procedures in the most natural-looking way and get the best outcome. Being the best version of themselves gives them confidence and improves their quality of life.”
The procedures drawing attention
Two procedures in particular have drawn increasing interest among male clients: jawline enhancement, often referred to at the clinic as the “Sexy Jaw,” and penile filler enhancement, known as “Power D.” Both are non-surgical and rely on injectable fillers.
Two procedures in particular have drawn growing interest among male clients: jawline enhancement, referred to at the clinic as the “Sexy Jaw”, and penile filler enhancement, known as “Power D.” Both are non-surgical and use injectables.
The “Sexy Jaw” treatment is primarily filler-based but may also combine Botox, fat-dissolving injections, and threads, depending on the client’s goal, face shape, and skin condition.
For the jawline, Dr. Cyril uses hyaluronic acid (HA), a substance naturally found in the body that is absorbable and FDA-approved, to create projection and definition in the lower third of the face.
“Many Asian men have a weak lower face, which can make the face look round and less masculine,” Dr. Cyril explains. “When you build structure in the lower face, it improves definition, proportion, and masculinity.”
The emphasis is on proportion rather than exaggeration. Dr. Cyril follows specific guidelines to avoid an overdone look.
“From the cheekbone to the jawline, it should be equal to or slightly smaller than the cheekbone, especially for Asian faces where slimmer contours are preferred. From the side, it should look angular but natural.””
He notes that unnatural results typically stem from over-injection. “When doctors over-inject, that’s when you see problems,” he says, citing patients who return too soon for additional volume. “Natural results require control, technical skill, and aesthetic judgment.”
Jawline fillers typically last about one to one and a half years, depending on the type of filler used and the movement of the treated area.
Function meets aesthetics
The clinic’s other high-demand procedure, Power D, uses fillers such as PDLA to enhance penile girth and length. PDLA is also FDA-approved and absorbable, but firmer and longer-lasting than HA, with results that can last up to two years.
“For the penis, the filler increases girth and at the same time length,” Dr. Cyril says. “It gives men more confidence and also improves performance. So the benefit is not just visual, but functional as well.”
He explains that the firmness of PDLA provides structural support. “Because of the firmness wrapping around the penis, there’s a sensation of constriction, which gives extra sensation,” he says. “Patients report they get firm or erect more easily. Sometimes, their partners notice the difference too.”
Safety and technique at its core
While the aesthetic outcome is important, both doctors emphasize safety and technique. HA is widely regarded as predictable and well tolerated. “We already have it in our body, and it’s absorbable,” Dr. Cyril says. “When you inject it, there’s barely any inflammation or swelling. It’s safe and FDA-approved.”
However, he stresses that safety ultimately depends on the injector. “Any product can be unsafe if used incorrectly. The doctor must know the anatomy, how the product behaves, and inject it correctly.”
Jan adds that the clinic invested significant time researching the fillers and protocols they offer. “We value quality and safety,” he says. He also notes that Dr. Cyril’s experience as an international trainer has helped refine techniques to ensure consistent and natural outcomes.
Patient selection is another critical factor. Absolute contraindications include patients on blood thinners, those with certain implants, or those who have undergone previous surgery in the treatment area. Beyond physical health, Dr. Cyril also assesses psychological readiness.
“I check for body dysmorphia, mental health issues, and whether they have a sexual partner,” he says. “I want to make sure the procedure is for the right purpose.”
Age is not a strict limitation. Younger patients often seek improved jawline definition, while older patients may pursue treatments for both aesthetic and functional reasons, including erectile dysfunction. The approach, however, is tailored.
“For mature clients, you want to improve and freshen without erasing lines that give wisdom and confidence,” Dr. Cyril says. “For younger clients, you avoid making them look older than they are.”
Confidence beyond appearance
Despite growing acceptance, stigma has not completely disappeared. Many male patients still arrive with visible hesitation. Jan observes that confidence shifts are often noticeable after treatment.
“Before treatment, many patients are shy and lack confidence,” he says. “Afterward, they become more sociable and happy. Some go back to dating. Some feel more confident at work.”
He argues that the impact extends beyond appearance. “Even small improvements can have a significant impact. These procedures are more than skin deep. They affect quality of life.”
Maintenance is relatively straightforward. Patients are advised not to massage the treated areas and to allow the filler to settle naturally. Healthy lifestyle habits, including proper sleep, limited alcohol intake, and regular exercise, can support healing and longevity. An active lifestyle, including workouts, does not negatively affect results.
Subtle enhancement, natural results
As male aesthetics continue to expand, the conversation appears to be shifting from secrecy to normalization. The focus, at least at clinics like Lift Aesthetic, remains on subtle enhancement rather than transformation.
For them, the guiding principle is simple:
“When men see results at Lift, it always looks natural. That’s why the process is far less intimidating than they expect.”
In an industry once dominated by female demand, the growing presence of male patients signals a broader recalibration of how masculinity and self-care intersect. Here, enhancement is less about appearance and more about confidence and control.

The Sexy Jaw procedure uses hyaluronic acid (HA) fillers to add subtle projection and definition to the lower face. It enhances jawline structure, improves facial proportion, and creates a more masculine appearance without surgery. Results typically last one to one and a half years.
Power D is a non-surgical penile enhancement using PDLA fillers to increase girth and length. The procedure also supports firmness and can improve sexual performance. Results can last up to two years.
Yes. HA and PDLA fillers are FDA-approved, absorbable, and well tolerated. Safety depends on proper injection technique, so treatments should be performed by a trained and experienced doctor.
Most men can safely undergo these procedures, regardless of age. Doctors screen for physical health, psychological readiness, and realistic expectations to ensure the treatment is appropriate. Men on blood thinners, with certain implants, or previous surgeries in the area may need to avoid treatment.
When performed by experienced practitioners, results are subtle and natural. The goal is proportional enhancement, avoiding overfilling, so patients appear refreshed or more defined without looking artificial.
